It is important that you feel comfortable with our office. Here is a list of tips intended to help prepare you for your appointment.
- Check with our staff regarding your health insurance benefits. While it is the patient’s responsibility to understand your benefits, we know how confusing that can be. Please contact one of our patient financial counselors who will be happy to help you.
- You should try and bring copies of any recent test, a list of current medication, allergies, x-rays, or other pertinent information to help our surgeons in your diagnosing and treatment of your condition. Please contact our office if you have questions or need help with this. Many times, if you can tell us where the test was performed, our staff will be able to get those records for you.
- Prescription refills are done Monday through Friday from 8:30 am to 4:00 pm. Please try and call two days before you run out.
- If you are a new patient, or have a new condition, arrive at least 20 minutes prior to your appointment. We will have paper work that we will need you to complete and this helps keep the office flow running smoothly. Our patient medical and demographic forms are below. To speed up your registration process, you can fill those out ahead of time.
